Job summary

Abraxas Youth and Family Services in Morgantown, PA is seeking a dedicated Case Manager to oversee a caseload of adjudicated youths and their families. The role focuses on transitioning clients from juvenile systems back into the community, with emphasis on coordinated service plans and thorough documentation.

The position requires completing court conditions, coordinating with multiple agencies, and conducting ongoing home visits and progress reporting. Weekend availability is expected.

Qualifications

• Bachelor’s degree in related field or equivalent experience.
• Two years of related juvenile field experience preferred.

Responsibilities

• Maintain a client caseload with appropriate service plans as needed.
• Coordinate and develop service plans utilizing identified resources to aid client transition from residential placement to the community.
• Monitor client files to ensure proper documentation and services; review information monthly.
• Supervise client activities and prepare reports on behavior and progress (daily, weekly, monthly).
• Support clients and families in completing court-ordered conditions (e.g., restitution, community service, counseling).
• Coordinate with courts, probation, schools, employers, and social services to meet client needs.
• Attend client court hearings, school meetings, and reviews; advocate as needed.
• Arrange weekly home visits with the client’s family and monitor hours spent with each family.
