Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or related field.
• Minimum of two years of relevant human services or case management experience.
• Intermediate computer proficiency, including experience with electronic documentation systems.
• Valid driver’s license and reliable transportation.
• Ability to pass federal, state, and local criminal background checks.

Responsibilities

• Participate as an active member of a multidisciplinary team from a holistic, values-based perspective.
• Conduct assessments and determine resources to meet participants’ needs.
• Develop, implement, and review recovery plans with participants and supports.
• Provide community-based case management focused on education, skills, and recovery goals.
• Advocate for participants to access services, supports, and community resources.
• Build and maintain relationships with community agencies for coordination.
