Job summary

Abraxas Youth and Family Services is seeking a dedicated Case Manager Supervisor to oversee a caseload of adjudicated delinquent youths and their families. The role focuses on transitioning clients out of the juvenile justice system, coordinating service plans, and ensuring compliance with licensing and contractual requirements.

Ideal candidates will bring strong team leadership, organizational skills, and a commitment to helping others.

Qualifications

  • Bachelor’s degree in related field and minimum of three years of related experience in the juvenile field; OR
  • Master’s degree in related fields and minimum of one-year related experience in the juvenile field.
  • Two (2) years of experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disability services required.

Responsibilities

  • Maintain a client caseload with appropriate service plans as needed.
  • Coordinate and develop service plans to assist client transition from residential placement to the community.
  • Monitor client files and ensure proper documentation and services.
  • Supervise client activities and prepare reports on behavior and progress.
  • Coordinate with courts, probation, schools, and other agencies to meet clients’ needs.
  • Attend client court hearings, school meetings, and reviews.

What You’ll Do

The position is responsible for a caseload of adjudicated delinquent youths and their families who are chronic in delinquent behavioral problems. The position is responsible for assisting the client to transition out of the juvenile justice system.


Your responsibilities may include:


  • Maintain a client caseload with appropriate service plans as needed.

  • Coordinate and develop appropriate service plans through the utilization of various identified resources to assist the client in making the transition from a residential placement/institution to the community in lieu of incarceration.

  • Monitor client files to ensure clinical staff maintain proper documentation and services, address performance issues, and review information on a monthly basis.

  • Supervise client activities and prepare required reports of the behavior and progress of clients. Daily, weekly, and monthly reports are required as well as month end reports, incident and arrest reports as needed.

  • Support clients and their families in completing any court ordered conditions (i.e. restitution, community service, and specific counseling needs and assessment).

  • Coordinate efforts with the courts, probation department, schools, employers, job training, counseling services, legal and medical services, or any other social service agencies and community programs to meet the clients’ individualized needs.

  • Maintain regular contacts with referral agency staff assigned to each client in order to maintain communication and consistency with client and family treatment and to meet contractual and licensing regulations.

  • Attend and act as advocate and/or liaison to all client court hearings, school meetings, and reviews.

  • Arrange and conduct home visits with the client's family on weekly basis throughout the referral. Meet required guidelines for number of hours spent with each client and family.

  • Facilitate groups for clients and families and assist in facilitating life skills workshops and seminars.

  • Provide information to external agencies and funding personnel regarding the program/facility mission statement and treatment philosophy.

  • Identify and utilize community-based resources to assist clients and families.

  • Provide transportation for clients as needed for program/facility related meetings and activities.

  • Supervises, assists with hiring, and evaluates staff performance, develops measurable objectives for necessary improvements and implements appropriate corrective action to staff adhering to human resources policies and utilizing Human Resources when necessary.

  • Develop a weekly work schedule and coordinate planning/scheduling to best service each client.

  • Assist program/facility supervisory staff members with scheduling staff to ensure adequate coverage at all times; review case files and consults on individual service plan; orient new staff and evaluate training needs.

  • Serve as a point of contact between appropriate clinical and administrative supervisory staff member.

  • Develop and maintain clinical client files for caseload.

  • Coordinate and develop appropriate service plans for clients to transition into the community through various identified resources.

  • Supervise and coordinate the electronic monitoring of clients and program/facility specific requirements include minimum four hours per week curfew checks.

  • Prepare service and aftercare plans for each client and monitor plans in a timely manner.

  • Supervise client activities and prepare necessary paperwork including daily, weekly, and monthly reports for internal and external use.

  • Perform groups and training for clients and appropriate staff members to include client curriculum and informative materials.

  • Supports the Abraxas philosophy and mission and promotes the Seven Key Principles of care.

  • Demonstrates appropriate use of Safe Crisis Management techniques and skills.

Qualifications

Education and Experience Required:



  • Bachelor’s degree in related field and minimum of three years of related experience in the juvenile field; OR

  • Master’s degree in related fields and minimum of one-year related experience in the juvenile field.


Experience:



  • Two (2) years of experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disability services required.

  • One (1) year of case management experience preferred.


Other Qualifications:



  • At least twenty-one (21) years of age

  • Must have a valid Driver's License

  • Physical exams include TB and drug testing.

  • Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.

  • Satisfactory completion of background screening and applicable pre-employment checks, including but not limited to employment and/or personal reference and driving records.

  • Ability to work with computers and the necessary software typically used by the department.


Schedule

Must be available to work weekends!
