Qualifications

• Bachelor’s or Master’s degree as specified.
• Two years of experience in behavioral health or juvenile field preferred.
• Must be 21+, valid driver's license, and able to pass background checks.

Responsibilities

• Maintain a client caseload with service plans as needed.
• Coordinate and develop service plans using identified resources to aid transition to the community.
• Monitor client files, ensure proper documentation, and review information monthly.
• Supervise client activities and prepare daily, weekly, and monthly reports.
• Coordinate with courts, probation, schools, employers, and social services to meet client needs.
• Attend hearings and meetings; conduct home visits weekly with families.
