Job summary

Abraxas Youth & Family Services is seeking a dedicated Case Manager to handle a caseload of adjudicated delinquent youths and their families, guiding transition from juvenile justice to the community. The role emphasizes collaboration with courts, schools, and social services to promote positive outcomes.

You will perform client supervision, documentation, and reporting, while coordinating service plans and ensuring compliance with licensing and program requirements.

Qualifications

• Bachelor’s degree in related field and at least three years of juvenile field experience.
• Master’s degree in related fields with at least one year of juvenile field experience.
• Two years of experience supporting individuals in behavioral health or related services.
• Valid driver’s license and ability to pass background checks.

Responsibilities

• Maintain a client caseload with appropriate service plans.
• Coordinate and develop service plans with community resources for transition from residential placement.
• Monitor client files; ensure proper documentation and monthly reviews.
• Supervise client activities and prepare weekly/monthly reports.
• Assist clients and families with court-ordered conditions and referrals.
• Coordinate with courts, schools, and community programs to meet client needs.
• Attend court hearings, school meetings, and reviews as advocate or liaison.
• Develop and maintain clinical files; supervise staff and plan scheduling.
