Lutheran Social Services of South Dakota is hiring a full-time Case Manager to support youth in the New Alternatives program. You will develop individualized plans, coordinate medical care, and connect clients with essential community resources.

The role requires a Social Work license or eligibility, a bachelor’s in social work or related field, and a commitment to safety and documentation. Benefits include extensive programs and career development.

Qualifications

• Bachelor’s degree in social work or related field required.
• Must be at least 21 years old.
• Licensed Social Worker or Licensed Social Work Associate preferred.
• Experience with Medication Administration Records (MARs) preferred.
• Willingness to undergo background checks to protect youth.
• Valid auto-insurance and driver's license or ability to obtain one.

Responsibilities

• Develop individualized case plans and conduct ongoing assessments; connect youth with community resources.
• Maintain accurate case documentation; build collaborative relationships with partners.
• Coordinate medical, dental, and vision appointments; ensure annual exams.
• Support medication management with nursing staff and pharmacy providers.
• Obtain medical records and treatment consents; review MARs for accuracy.
• Transport clients to appointments as needed.
