Job summary

Child & Family Service in Ewa Beach, Hawaii, is seeking a Case Manager for the Positive Pathways Youth Mentoring Program. You will work under supervision to provide guidance, encouragement, and trauma‑informed support to at‑risk youth, coordinating services and developing youth and family driven plans.

The role requires outreach, screening, intake, and comprehensive assessments, plus collaboration with mentors, schools, and partners.

Qualifications

• High school diploma plus additional courses or training in business or community college or similar program.
• Bachelor’s Degree preferred; human services licensing or apprenticeship may be preferred.
• Two years of experience with at‑risk youth; preferably with juvenile justice involvement.

Responsibilities

• Outreach, screening, intake, and comprehensive assessments for youth and families.
• Develop individualized service plans reflecting strengths and needs.
• Coordinate services and resources to support positive outcomes.
• Provide trauma-informed case management, advocacy, and care coordination.
• Collaborate with mentors, schools, and community partners; participate in home or school visits as needed.
