Resources for Human Development in Norristown, PA is seeking a dedicated Case Manager to coordinate care, maintain treatment plans, and support youth through a collaborative, treatment-focused approach. This full-time role offers training, growth opportunities, and a supportive team.

You’ll work with clinicians, families, and providers to ensure access to services, maintain accurate records, and help youth achieve positive outcomes with compassionate, person-centered care.

Qualifications

• High School Diploma or equivalent.
• Strong written and verbal communication skills.
• Ability to maintain accurate documentation and electronic records.
• Strong critical thinking and problem-solving abilities.
• Ability to complete required trainings and certifications, including CPR/First Aid, Medication Administration, Crisis Intervention, Trauma-Informed Care, HIPAA, Mandated Reporting.

Responsibilities

• Coordinate medical care and treatment services for youth in care.
• Maintain medical records and ensure compliance with regulatory requirements.
• Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments.
• Assist with transportation to medical appointments as needed.
• Monitor medication administration and complete weekly medication counts.
• Train staff in medication administration, CPR, and First Aid procedures.
• Communicate medical updates and treatment information with parents, guardians, providers, and treatment team members.
• Collaborate closely with clinicians, direct care staff, families, and community partners to support positive outcomes.
• Maintain accurate documentation and electronic records.
• Assist with program operations and provide support coverage when necessary.
• Support quality care initiatives and contribute to overall program success.
• Perform additional duties as assigned.
