Job summary

Elizabeth Layton Center, Inc. in Paola, KS is seeking 2 Children's Case Managers to join our CBS team. The role focuses on case management and therapeutic services for youth with SED, primarily in family homes and community settings.

Responsibilities include treatment planning, resource coordination, and advocacy across Education, Residential, and Health domains. A driving record is required, and Medicaid security checks will be completed.

Qualifications

• Bachelor’s degree or equivalent experience to meet Kansas licensing.
• Meets Kansas Case Management Program Standards.
• Familiarity with mental health theory and practice.
• Clean driving record.
• Must pass Medicaid security check.

Responsibilities

• Participates in treatment planning and applies interventions.
• Develops personal plan/goal sheet with consumer needs.
• Obtains location and process of acquiring resources; coordinates with area agencies.
• Direct involvement in planning/advocacy in Education/Vocation, Residential, Financial, Health, Leisure and Social Supports.
• Educates family on symptomatology, relapse indicators and crisis handling.
• Provides program data and reports to immediate supervisor.
• Works with other mental health center staff and maintains required documentation.
