The role focuses on providing case management and therapeutic services to consumers with severe and persistent mental illness, primarily in home and community settings. Under the CSS Director, you will develop individual plans, coordinate resources, educate families, and advocate across education, housing, health, and social supports.

Qualifications

• Bachelor’s degree or equivalent experience to meet Kansas licensing
• Familiarity with mental health theory and practice
• Valid driver’s license and clean driving record
• Ability to pass required background checks

Responsibilities

• Participates in treatment planning based on best practice theories and applies interventions
• Develops personal plan/goal sheet with consumer needs
• Obtains resources and coordinates with area agencies
• Advocates in Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports
• Educates families on symptomatology and relapse indicators
• Collects program data and provides reports to supervisor
• Provides transportation to program activities as needed
• Meets deadlines for daily paperwork and reports
• Documents accurately and maintains quality records
• Assists other case managers and consumers as needed
