Job summary

The Elizabeth Layton Center in Paola, KS is seeking 2 qualified Children’s Case Managers to join our CBS (Community Based Services) team. The role focuses on providing case management and therapy to youth with severe emotional disturbance in home, school or community settings.

You will collaborate with therapeutic staff, develop plans, coordinate resources, and advocate for family education and skills development to promote independence and community inclusion.

Qualifications

• Bachelor’s degree or equivalent experience to meet State of Kansas licensing requirements.
• Meets Kansas Case Management Program Standards.
• Familiarity with mental health theory and practice.
• Clean driving record.
• Must pass security check as required by Medicaid regulations.

Responsibilities

• Participates in treatment planning based on best practices and applies interventions.
• Develops personal plan/goal sheet with consumer needs.
• Obtains location for resources and coordinates with area agencies.
• Direct involvement in planning and advocacy in Education/Vocation, Residential, Financial, Health, Leisure and Social Supports.
• Educates family on symptomatology and crisis handling.
• Collects program data and provides reports to supervisor.
• Works with other mental health center staff.
• Provides transportation to program activities as needed and maintains driving record.
• Meets direct service expectations of 47% of time worked.
• Works evening hours as needed.
• Meets deadlines for daily paperwork and reports.
• Provides quality documentation and supports after-hours needs.
