AMIkids, Inc. is seeking a Case Manager to oversee service planning and transitional processes for youth entering and exiting the program.

You will tailor individualized plans for each youth and monitor progress to ensure effectiveness, aligning with the organization’s mission to empower young people. The role involves intake assessments, reassessments, and transition planning, collaborating with the MDT, delivering evidence-based care, and attending court hearings when required.

Formación

• Bachelor’s degree in psychology, social work or related field.
• Minimum 2 years’ experience working with youth.
• Ability to develop and monitor individualized service plans for youth.

Responsabilidades

• Perform intake needs assessments, re-assessments and transition planning.
• Develop and document individualized service/care plans based on needs assessments and update as needed.
• Actively participate in the Multidisciplinary Treatment Team (MDT).
• Deliver evidence-based treatment models with fidelity and proper documentation.
• Attend and represent the program at court hearings and youth-related proceedings.
