Job summary

AMIkids is seeking a Case Manager to oversee service planning and transition processes for youth entering and exiting the program. You will develop individualized plans and monitor their effectiveness, coordinating with MDT and ensuring fidelity to evidence-based treatment models.

You will participate in court hearings and meetings, supporting youth through intake, reassessments, and transition planning as part of our mission to empower young people and foster their potential.

Qualifications

• Bachelor's degree in psychology, social work or related field.

Responsibilities

• Complete intake needs assessment, re-assessments, and transition planning.
• Evaluate, develop and document an individual service/care plan for the youth based on the needs assessment, and complete required updates.
• Active participant in the Multidisciplinary Treatment Team (MDT).
• Deliver Evidence Based Treatment models adhere to fidelity requirements of evidence-based treatment model fidelity and documentation.
• Attend and represent the program at court hearings, meetings and youths' legal proceedings.

Education

What you will be doing:
