Job summary

Wedgwood Christian Services in Grand Rapids, MI seeks a Case Manager to support delivery of case management and clinical treatment services. You will coordinate services for youth, maintain records, and liaise with families and sponsors to ensure seamless care across providers.

Requirements include a bachelor’s degree with preference for LBSW licensure, bilingual English/Spanish, and at least one year in case management or youth treatment roles.

Qualifications

• Bachelor's degree is required.
• LBSW or ability to be licensed is preferred.
• Prior experience working with refugee and/or immigrant populations preferred.
• Minimum of one year experience as a Case Manager or a Youth Treatment Specialist.
• Excellent verbal and written communication skills.
• Bi-lingual English and Spanish preferred.

Responsibilities

• Ensure all youth receive services across mental health, medical, recreation, education, and other supports.
• Maintain current information on foster children and meet ORR reporting requirements.
• Coordinate family connections and sponsor involvement.
• Assist Clinician/Consultant with scheduling activities and referrals.
• Assist with Case Reviews by sending invitations, compiling results, attending meetings, and writing summaries.
• Process case information, prepare profile sheets for admissions, and follow-up on discharged clients.
• Maintain liaison with Goal Worker and nurses, wellness staff, school coordinator, ensuring updates across providers.
• Lead or co-lead a milieu group when requested.
• Monitor progress of select clients, including community living skills.
• Participate in supervision and team meetings for planning.
• Ensure quality assurance compliance and timely reporting.
• Other duties as assigned.
