Job summary

Catholic Charities West Michigan is seeking a Transitional Foster Care Case Manager in Grand Rapids to coordinate case management and safe release services for youth in transitional foster care. You will manage admissions, assessments, reunification planning, service coordination, and documentation with foster families, sponsors, and providers.

Qualifications

• Bachelor's degree in behavioral sciences, human services or social services fields required.
• Minimum 1 year of experience in child and family services or counseling encouraged.
• Bilingual Spanish/English fluency highly preferred.
• CPR and First Aid certification upon hire or within 60 days is preferred.

Responsibilities

• Coordinate intake assessments within 24 hours and orient youth to program.
• Maintain regular contact with youth and provide weekly updates on needs and reunification progress.
• Identify and engage potential sponsors; document all contacts and case reviews.
• Collaborate with sponsors, families, and providers to support timely reunification and permanency planning.
• Ensure on-site, confidential delivery of child-facing services and complete required documentation.
