Job summary

Youth Outreach Services (YOS) is seeking a compassionate Case Manager to support families involved with the child welfare system. The role emphasizes assessment, case management, advocacy, and connecting clients to community resources to promote family stability in Chicago.

Responsibilities include home visits, developing service plans with families, documenting timelines, and coordinating with schools and service providers. A valid Illinois driver's license is required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field is required.
• Experience working with youth and families is preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Manage a caseload of up to 10 families.
• Complete assessments and required documentation within established timelines.
• Develop service plans and goals in collaboration with clients and families.
• Conduct regular home visits and provide ongoing support services.
• Assist families in accessing community resources and supportive services.
• Maintain relationships with referral sources, schools, service providers, and community partners.
• Prepare court reports and testify in court when required.
• Participate in supervision, team meetings, and agency trainings.
• Ensure compliance with DCFS, agency, and confidentiality requirements.
