Job summary

SGA Youth in Chicago seeks an Outreach Specialist to recruit and maintain a caseload of up to 50 participants and conduct monthly contacts. You will build trust with families and community partners, promote Healthy Start services, and navigate referrals to clinics and local organizations.

The role requires a Bachelor's in Social Work or related field, experience with mothers and families, and strong cultural competency. Flexibility for evenings/weekends and driving to client sites is expected.

Qualifications

• Bachelor’s degree in Social Work, Early Childhood or related field required.
• Experience working with mothers, fathers, and families.
• Experience in case management.
• Bilingual English/Spanish required.

Responsibilities

• Recruit and maintain a caseload of up to 50 participants with monthly contact.
• Establish and maintain cooperative relationships with families and community organizations.
• Provide home-based health education, lactation support, and case management activities.
• Assist participants with creating and updating an Individual Family Service Plan quarterly.
• Promote father/partner engagement and immunization awareness.
• Ensure HIPAA compliance and maintain client records.
