Job summary

Catholic Charities is partnering with local agencies to deliver case management and outreach services for at-risk youth and their families. The role focuses on connecting families with resources, facilitating access to services, and supporting positive outcomes within a team-based environment.

The position emphasizes autonomy, collaborative work with schools and community partners, and adherence to program guidelines.

Qualifications

• BA preferred or extensive equivalent experience.
• Bilingual is a plus.
• Experience in case management/counseling in a community-based setting.
• Ability to work autonomously and in a team.
• Strong verbal and written communication skills.
• Fingerprinting and background check as required by DOJ/DMV clearance.

Responsibilities

• Researches and has knowledge of community resources.
• Provides needs assessment and linkage for case management.
• Links families to resources to promote well-being and mental health.
• Assist families with service navigation and documentation.
• Maintain and organize client records and reports (Excel).
• Provide outreach and program support as needed.
• Meet weekly with supervisor to discuss cases and program needs.
• Provide after-hours support by phone when needed.
• Advocate for clients in school, court, or probation contexts.
