Job summary

Catholic Charities of the Archdiocese of Chicago is seeking a Case Manager - Empowering Families to conduct comprehensive client assessments, develop individualized service plans, provide advocacy and resources, and ensure timely documentation in line with CCAC policies.

You will collaborate with care teams, engage in outreach, and pursue ongoing learning to improve client outcomes while supporting families and communities across Cook and Lake counties.

Qualifications

• Associate's Degree in a related field; 1–3 years relevant experience.
• Excellent written and verbal communication skills.
• Strong interpersonal skills.
• Ability to work effectively both independently and as part of a team.

Responsibilities

• Conduct comprehensive assessments to determine client needs and develop service plans.
• Provide direct support, guidance, and access to services (housing, employment training, counseling).
• Maintain accurate documentation and advocate for clients within the community.
• Collaborate with multidisciplinary teams to coordinate services and conduct outreach.
• Engage in ongoing learning and apply feedback to improve performance.
