The Puerto Rican Cultural Center in Chicago is looking for a Case Manager to provide direct services to the target population. The role requires individual case management, follow-up services, and career counseling.

Qualified candidates should possess at least 2 years of experience in youth programs or case management and be proficient in both English and Spanish. This full-time position offers a salary of $45,000 – $50,000 along with health, vision, dental benefits, and PTO.

Competenze

• 2 years of youth program and/or counseling experience required.
• Ability to work independently with minimal guidance.
• Task-oriented, non-judgmental, and clear sense of boundaries.

Mansioni

• Provide individual case management and career counseling.
• Support development of service plans for clients.
• Determine client eligibility for program services.
