Job summary

ChangePoint Integrated Health Centers in Arizona seeks a BHT/Case Manager to advocate for clients, obtain entitlements, and coordinate transportation between ChangePoint and other service providers. You will help develop and implement treatment plans and maintain clinical documentation while ensuring productive billable hours.

Qualifications

• Fluent in English, both verbally and in writing.
• Literate and able to complete difficult paperwork in a timely manner.
• Experience with analytical and problem-solving skills.
• Ability to prioritize and handle multiple tasks with customers.
• Proficient in charting and record keeping to DHS standards.
• 5+ years Behavioral Health experience for BHT case management level.

Responsibilities

• Adheres to agency policies and ethical standards.
• Provides case management and coordinates client entitlements.
• Manages clinical records, crisis services, and service coordination.
• Engages clients in treatment plan development and implementation.
• Maintains confidentiality and accurate documentation.
• Provides transportation and community resource linkage as needed.
• Coordinates wrap services and, when needed, home/school visits.
• Participates in quality management and safety initiatives.
