Mychangepoint's SAFE House is seeking a dedicated BHT/Case Manager to advocate for residents, secure entitlements, and coordinate transportation with service providers. The role emphasizes collaboration to create a therapeutic and safe environment that supports emotional growth.

You will develop and implement residents' DV education programs, maintain compliant clinical records, and actively participate in quality management and training to uphold state standards.

Formación

• Fluent in English, both verbally and in writing.
• High School Diploma required.
• 5 years Behavioral Health Experience for BHT case management level.
• Current Arizona Driver's License.
• Comply with a Drug Free workplace policy.
• Must be at least 21 years old.

Responsabilidades

• Advocate for clients and obtain entitlements.
• Coordinate transportation between SAFE House and service providers.
• Maintain clinical records and documentation.
• Provide crisis services and coordinate service delivery.
• Engage residents in goals planning and weekly review of progress.
• Assist with housing and resource referrals and community training.
