ChangePoint is seeking a dedicated BHT/Case Manager to advocate for clients, coordinate entitlements, and arrange transportation between ChangePoint and service providers.

The role emphasizes collaborative teamwork, documenting clinical activity, and maintaining productivity through billable hours while delivering comprehensive supports to clients across behavioral health, housing, income, and health services.

Formación

• Fluent in English, both verbally and in writing.
• Literate and able to complete paperwork accurately.
• Analytical and problem-solving abilities required.
• Ability to prioritize and handle multiple tasks for customers.
• Proficient in producing professional documents and charting in customers’ charts.

Responsabilidades

• Advocates for clients and provides case management services.
• Maintains clinical records and provides crisis services as needed.
• Coordinates transportation and services with ChangePoint and other providers.
• Engages clients in development and implementation of treatment plans.
• Assists clients in obtaining entitlements and benefits.
• Provides health promotion groups and psycho-social support.
• Works with multiple agencies to coordinate wrap services.
