Qualifications

• Experience using case management systems to enter notes, track client information, and monitor performance.

Responsibilities

• Conduct and update individual/family needs assessments to develop an individualized service plan.
• Provide two-generational, family-driven, and strength-based services to youth and families.
• Coordinate and monitor services to meet program participant needs and goals.
• Maintain accurate documentation of services and provide data for program evaluation.
• Attend and represent Agape in meetings and training with partner agencies.
• Be flexible with schedule, including evenings and weekends.
