Qualifications

• Ability to work with diverse populations and demonstrate culturally competent service delivery.
• Excellent written and verbal communication, and data documentation skills.

Responsibilities

• Assess student and family needs; develop individualized service plans.
• Coordinate supports across health, mental wellness, housing, education, and wraparound services.
• Document case notes and data; maintain accurate reports and input into data systems.
• Collaborate with schools, families, and community partners to advance program goals.
• Attend meetings, trainings, and supervision sessions; participate in program quality improvement.
