Job summary

Agape is seeking a Family Connector to advance mission through a hope-centered, two-generation approach, working in schools, neighborhoods, and communities to build authentic relationships with families and provide individualized support and resource navigation.

This full-time, community-based role requires collaboration with internal teams to ensure quality service delivery, accurate documentation, and meaningful outcomes, with flexible scheduling including evenings and weekends.

Qualifications

• Bachelor's degree in Social Work, Education, or related field.
• Minimum of two years of relevant experience in case management, family support, community-based services, or related fields.
• Demonstrated experience working with diverse populations and delivering culturally responsive services.
• Strong written and verbal communication skills.
• Proficiency in Microsoft Office 365 applications and ability to learn additional systems.
• Understanding of data collection, reporting requirements, and outcome measurement.

Responsibilities

• Build trusting, strengths-based relationships with children, youth, caregivers, and families through engagement and support.
• Connect families to educational, workforce development, mental health, healthcare, housing, and other services; coordinate ongoing support.
• Apply hope-centered principles to empower families toward actionable goals and flourishing.
• Collaborate with schools and community partners to maximize resources and coordinated services.
• Maintain accurate documentation and contribute to PQI processes and program improvement.
• Participate in team meetings and organizational activities to support program priorities.
