Job summary

Agape Child & Family Services’ STARS Connector in Memphis, TN, seeks a compassionate professional to manage a school-based caseload, align services with the Science of Hope and Human Flourishing, and bridge students and families to community resources.

You will conduct needs assessments, coordinate wraparound supports, document progress, and participate in team meetings while working a flexible schedule including evenings and weekends.

Qualifications

• Bachelor’s degree in education or social work.
• Demonstrated case management experience.
• Strong communication and intercultural competence.

Responsibilities

• Assess student/family needs and develop individualized service plans.
• Provide two-generational, strength-based services.
• Coordinate wraparound supports: health, housing, education, employment.
• Record data and maintain documentation for program reporting.
• Engage families with schools and community partners.
• Attend meetings and participate in training.
• Support during summer programs and events.
• Maintain attendance, behavior, and parental engagement goals.
• Work with Powerlines Community Network partnerships.
