Job summary

Qualifications

• Bachelor’s degree in Social Work or related discipline required.
• Six months experience in social services preferred; strong written and oral communication.
• Proficient with Microsoft Office and data collection/analysis software.
• Valid driver’s license required; ability to coordinate with families and stakeholders.

Responsibilities

• Coordinate care and communicate treatment plans to families and guardians.
• Participate in client treatment team meetings and report on engagement.
• Complete documentation for family engagement, care coordination and discharge summaries.
• Liaise with external stakeholders and child welfare agencies.
