Job summary

Devereux is seeking a Care Coordinator in West Chester, PA, to join the treatment team and ensure quality services for children with behavioral and emotional support needs. The role focuses on coordinating family engagement, communicating treatment progress, and supporting discharge planning.

Qualifications

  • Six months experience in Social Service preferred; proficient computer skills and data collection/analysis experience.
  • Valid driver's license must be maintained.

Responsibilities

  • Welcome families/guardians to the Devereux Family and relay expectations for engagement.

  • Participate in client treatment team meetings and report on family engagement and discharge planning.
  • Complete documentation related to family communication and care coordination activities.

At Devereux as a Care Coordinator you will impact the individuals you are working with by ensuring quality services are delivered, and all team members, family members and external stakeholders are aware of the students progress in treatment. The Care Coordinator is a full time position working within a Psychiatric Residential Treatment Facility (PRTF) for Children ages 8-21 years with behavior and emotional support needs located in West Chester, PA.

Every day as a Care Coordinator you will....

  • Welcome families/guardians to the Devereux Family, relaying expectations for family engagement/involvement while child is in care and completing a Bio-psychosocial assessment
  • Communicate with families/guardians the treatment and services being provided to their child
  • Participates in client treatment team meetings and reports on family engagement, supervised family visitation, family court, and care coordination activities that facilitate current treatment and discharge planning
  • Complete documentation related to family communication/engagement and care coordination activities, and discharge summaries

  • BENEFIT ELIGIBILITY AFTER 30 DAYS OF EMPLOYMENT (1st day of the 1st pay period after 30 days).
  • Hourly rate of $19.66-$22.41

Devereux Pennsylvania Children's Services (DPACS) center serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Located in West Chester and Malvern Pennsylvania, services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives.

Qualifications
EDUCATION:

Bachelor's degree in Social Work or related discipline required.

EXPERIENCE:

Six months experience in Social Service preferred. Proficient computer skills, mastery of Microsoft Office Applications, experience with data collection/analyzing platforms (Radar, Pro-Filer, and/or other internal and external software applications pertinent to the position). Strong organizational skills. Excellent oral and written communication skills. Requires ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines. Valid driver's license must be maintained.

PHYSICAL REQUIREMENTS:

Ability to administer CPR. Ability to administer first aid. Ability to properly restrain adolescent/adult client. Ability to create and maintain a therapeutic milieu. Regularly requires sitting (1/2 - 3/4 of the work day). Often requires walking (1/4 - 1/2 of the work day). Seldom requires standing, stooping/bending, lifting 5 to 20 pounds and pushing/pulling (0 - 1/4 of the work day). Often requires use of office equipment often requiring repetitive wrist/hand movement.
