Job summary

Devereux is seeking a Care Coordinator in West Chester, PA, to join the treatment team and ensure quality services for children with behavioral and emotional support needs. The role focuses on coordinating family engagement, communicating treatment progress, and supporting discharge planning.

The position is full-time within a Psychiatric Residential Treatment Facility, with responsibilities that include documentation, collaboration with external stakeholders, and continual program improvement.

Qualifications

• Bachelor's degree in Social Work or related discipline required.
• Six months experience in Social Service preferred; proficient computer skills and data collection/analysis experience.
• Valid driver's license must be maintained.

Responsibilities

• Welcome families/guardians to the Devereux Family and relay expectations for engagement.
• Communicate with families the treatment and services being provided to their child.
• Participate in client treatment team meetings and report on family engagement and discharge planning.
• Complete documentation related to family communication and care coordination activities.
