Qualifications

Responsibilities

• Coordinate services and referrals between health clinics and HealthConnect.
• Assist participants with social issues such as houselessness, food insecurity, and access to benefits.
• Arrange follow-up appointments, prescriptions, and connections to specialty health as needed.
• Coordinate transportation and escort participants to appointments.
• Maintain accurate records of interactions and referrals in CCS.
