Job summary

Memorial Healthcare is seeking a community program coordinator to provide case management, care coordination, and informal counseling for youth and families enrolled in the program. You will evaluate client needs, develop daily activity schedules, arrange transportation to appointments, and report outcomes to the Team Leader, while maintaining a flexible, high-communication approach in a busy environment.

Requires strong accountability, accuracy, and the ability to work with minimal supervision;

Qualifications

• Bachelor's degree required.
• Must have valid Florida driver's license and reliable personal vehicle.
• Basic Life Saving (BLS) certification within six months of hire.

Responsibilities

• Provides case management, care coordination, and informal counseling for clients and families enrolled in the program.
• Evaluates and assesses client/family needs and develops daily activity schedules.
• Transports clients to appointments and community resources as needed.
• Completes surveys/assessments and reports outcomes to the Team Leader.
