Behavioral Health Care Partners (BHP) seeks Family Care Coordinators to provide care coordination services to youth and their families, birth through age 22 if still enrolled in school, and experiencing mental health and/or substance use concerns. Responsibilities include coordinating with families, schools, and community resources, monitoring progress, and maintaining documentation.

Behavioral Health Care Partners (BHP), formerly Moundbuilders Guidance Center, is seeking Family Care Coordinators to provide care coordination services to youth and their families. This position works with youth from birth through age 22 if still enrolled in school and are experiencing mental health and/or substance use concerns.
