Job summary

Integrated Services for Behavioral Health is seeking a CANS Assessment Associate to conduct comprehensive CANS assessments for youth across virtual, home, school, and community settings. You will coordinate with families and partners to connect youth to services, maintain accurate documentation in the EHR and CANS portal, and ensure compliance with HIPAA and state regulations.

• High school diploma or GED required.
• Current Ohio CANS certification or ability to certify within 21 days.

Responsibilities

• Conduct comprehensive CANS assessments for children, adolescents, and transitional-age youth in virtual, home, school and community settings.
• Know OhioRISE referral pathways and program options (1915C waivers, YFC/Ifast/IHBT, QRTP requests).
• Maintain accurate records in the electronic health record (EHR) and CANS portal.
• Collaborate with families, providers and community partners to translate assessment findings and next steps.
• Build and maintain relationships with referral systems and collateral agents in the CANS or OhioRISE care umbrella.
• Adhere to HIPAA, safety rules, and professional guidelines; ensure timely documentation.
• Travel within designated service areas as required.
