Job summary

Qualifications

Responsibilities

• Conduct CANS assessments for children, adolescents, and transitional‑age youth in virtual, home, school, and community settings.
• Coordinate with families, providers and community partners to translate assessment findings.
• Maintain accurate documentation and ensure compliance with agency, state, and OhioRISE requirements.
• Travel within designated service areas and build collaborative referral relationships.
• Maintain and utilize EHRs and CANS portal for timely client records.
