Job summary

Qualifications

• High school diploma or GED required; Ohio CANS certification within 21 days.
• 2–3 years of experience in community mental health or related human services.
• Strong communication, organization, and data/documentation skills with HIPAA awareness.

Responsibilities

• Conduct CANS assessments for children, adolescents, and transitional-age youth in virtual, home, school, and community settings.
• Coordinate with families, providers, and community partners to plan care and next steps.
• Maintain accurate documentation in EHR and CANS portal, ensuring compliance with state and OhioRISE requirements.
• Collaborate with clinical staff to translate assessment findings into appropriate services.
• Travel within designated service areas; participate in cross-system referrals.
