Job summary

Rimrock Trails Treatment Services in Prineville, OR is seeking a Medical Care Coordinator for a 32-hour workweek in a residential program. You will coordinate medical care, medication management, and discharge planning to ensure safe, timely care for youth.

The role operates within MA scope of practice under supervision and emphasizes collaboration with therapists and clinicians to support coordinated treatment. Competitive compensation is offered.

Qualifications

• Current Medical Assistant (MA) certification is required.
• Experience providing direct services, care coordination, medical coordination, medication support, behavioral health support, or administrative healthcare coordination in a residential or youth-serving setting.
• Ability to work within clearly defined protocols and scope of authority.
• Strong attention to detail, organization, follow-through, and confidentiality.
• Ability to provide exceptional customer service and work with diverse stakeholders.
• Ability to work individually and as part of a multidisciplinary team.
• Hold a valid Oregon driver's license (if transportation duties are assigned).

Responsibilities

• Coordinate medical, dental, vision, and other healthcare needs for youth in residential treatment.
• Maintain medication histories and care coordination documentation.
• Coordinate prescription refills and provider follow-up within established protocols.
• Support discharge planning and continuity-of-care activities post-discharge.
• Serve as a central point of contact for healthcare-related communication among youth, families, staff, providers, and leadership.
• Provide training to residential staff on medication coordination and safety.
• Ensure documentation meets regulatory and compliance requirements.
