Job summary

Hope For Youth in Bay Shore, NY is seeking a dedicated Therapist to join our Youth Assertive Community Treatment (ACT) Program.

As part of an interdisciplinary team, you will provide trauma-informed therapy in home, community and office settings, develop individualized treatment plans, and coordinate with families, DSS and schools to restore functioning.

This full-time role offers competitive pay, benefits, and the New Hire Bonus of $1,000 plus a $1,500 bilingual bonus.

Qualifications

• Licensed mental health professional (LMSW/LCSW/LMHC/LMFT) with limited permits.
• Minimum 1 year of experience with children/adolescents and families preferred.
• Proficient in MS Office and electronic health records (EHRs).
• Valid NYS driver's license in good standing.
• Trauma-informed, culturally and linguistically competent service delivery.

Responsibilities

• Function within program policies and procedures.
• Adhere to state regulations and code of conduct.
• Collaborate with an interdisciplinary team to support client functioning.
• Conduct family, individual, and group therapy in home, community, and office.
• Ensure trauma-informed, culturally and linguistically competent service delivery.
• Attend trainings and supervision as assigned.
• Liaise with DSS, schools, and other contacts.
• Attend daily team and family meetings.
• Conduct comprehensive clinical assessments to diagnose needs.
• Develop treatment plans to restore functioning.
• Apply Evidence-Based Interventions to meet client needs.
• Provide transportation for clients as needed.
• Support crisis management and off-hours coverage.
• Perform other duties to meet program requirements.
