Job summary

Liberty Resources Integrated Health Care is seeking a compassionate, licensed Community-Based Therapist to join our Youth ACT team. This role provides intensive, community-based behavioral health services to youth with Serious Emotional Disturbance in homes, schools, and community settings, emphasizing trauma-informed, family-driven practices.

Qualifications

• Master's degree in Social Work, Mental Health Counseling, MFT, Psychology.
• Current New York State license (LCSW, LMSW, LMHC, LMFT, or Psychologist).
• Experience with children, adolescents, and families experiencing behavioral health challenges.
• Strong assessment, treatment planning, crisis intervention, and care coordination skills.
• Knowledge of trauma-informed care and evidence-based treatment approaches.
• Excellent communication, documentation, and organizational skills.
• Valid New York State driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive behavioral health assessments and develop treatment plans.
• Provide evidence-based individual, family, and group therapy in home, school, and community settings.
• Deliver crisis intervention, risk assessments, safety planning, and ongoing clinical support.
• Collaborate with psychiatrists, nurses, care managers, schools, healthcare providers, and community agencies to coordinate services.
• Promote youth engagement, family participation, skill development, and successful community integration.
• Participate in multidisciplinary treatment team meetings and shared caseload management.
• Complete timely, accurate clinical documentation in compliance with OMH, Medicaid, HIPAA, and agency standards.
• Participate in required trainings, supervision, quality improvement initiatives, and fidelity monitoring.
• Participate in rotating after-hours on-call coverage as required.
• Perform other related duties as assigned.
