Job summary

CN Guidance & Counseling Services, Inc. seeks a qualified clinician to work with the Youth ACT team to provide assessment, planning, and evidence-based services for youth with severe emotional disturbance and their families.

The role emphasizes culturally sensitive, community-based treatment and ongoing coordination with partner agencies. Responsibilities include documentation, QA processes, 24/7 availability as needed, discharge planning, and staff training to maintain compliance with OMH

Qualifications

• Master's degree in Social Work with Limited Permit required.
• Two years of experience with chronically mentally ill adults.
• Competency in substance abuse, family support, or rehab services demonstrated.

Responsibilities

• Provide Assessment and Service Planning and ensure quality improvement.
• Deliver culturally sensitive, developmentally appropriate treatment to youth and families.
• Maintain case records and complete documentation on time per regulations.
• Offer in-person, community-based services and adjust to needs in real-time.
• Coordinate with Youth ACT and other agencies to ensure compliance and inform clients.
• Participate in staff development, supervision, and NYS Youth ACT trainings.
