Job summary

CN Guidance & Counseling Services is seeking a qualified social worker to join the Youth ACT Team in New York. The role focuses on delivering intensive, community-based services for youth with severe emotional disturbance and their families, including assessment, planning, and therapy.

You will work under OMH guidelines, participate in team trainings, and coordinate with partner agencies. A Master's in Social Work with Limited Permit and relevant experience are required for this full-time

Qualifications

• Master's degree in Social Work with Limited Permit is required.
• Two years' experience working with the chronically mentally ill (Masters level)
• Competency in substance abuse, family support, or vocational/educational rehabilitation demonstrated via training/experience.

Responsibilities

• Provide Assessment and Service Planning, QA and Improvement, Utilization Review, incident reporting, and coordination with Youth ACT.
• Provide culturally sensitive treatment to Youth and families (e.g., Assertive Engagement, Case Management, Counseling).
• Maintain case records and timely documentation per regulations.
• Provide intensive, in-person community-based services aligned with OMH regulations.
• Possible after-hours availability and transportation for clients as needed.
• Support discharge planning and linkage to resources post-treatment.
