Job summary

CN Guidance & Counseling Services, Inc. delivers person-centered integrated clinical treatment, counseling, housing, and support services to youth with severe emotional disturbance and their families.

The role emphasizes assessment, planning, and intensive community-based services tailored to each individual’s needs, with ongoing coordination across systems. We seek a clinician with a master's degree in social work, limited permit, and at least two years of relevant experience.

Qualifications

• Master's degree in Social Work with Limited Permit (required).
• Two years' experience with chronically mentally ill (Masters level).
• Competency in substance abuse, family support, or vocational/educational rehabilitation—demonstrated by at least one year of training/experience.

Responsibilities

• Provide Assessment and Service Planning, QA and Improvement, Utilization Review, incident reporting, and coordination with Youth ACT and other systems.
• Deliver culturally sensitive, developmentally appropriate treatment to Youth and families through various therapies and services.
• Maintain case records and documentation in compliance with regulations.
• Provide intensive, in-person, community-based services tailored to individual needs with real-time adjustment.
• Be available 24/7 for coverage as needed by Youth ACT Team.
