Qualifications

• Master’s Degree required – MHC-LP, LMHC, LMSW, LCSW, or NYS licensure preferred.
• At least one year of experience in human services with individuals with serious mental illness.
• Ability to work in a fast-paced environment with deadlines.

Responsibilities

• Conducts daily community visits and documents progress notes per agency/ACT/OMH standards.
• Maintains and updates initial assessments, Service Plans, and 6-month Comprehensive Assessments.
• Conducts individual psychotherapy using evidence-based modalities.
• Participates in daily meetings and informs ACT team of family issues and treatment findings.
• Involves family and others in treatment; provides psycho-education and relapse prevention.
• Act as advocate for consumers with family, housing providers, and community supports.
• Provide 24-hour crisis intervention on-call services on rotation.
• Performs other duties as directed by Team Leader.
