Job summary

CN Guidance and Counseling Services, Inc. is seeking a skilled clinician to join the Youth ACT Team in delivering services to youth with severe emotional disturbance and their families.

The role emphasizes assessment, service planning, crisis intervention, and integrated mental health and substance abuse care. Requirements include a Master’s degree in Social Work with Limited Permit and two years’ experience with chronically mentally ill populations.

Qualifications

• Master's degree in Social Work with Limited Permit.
• Two years' experience working with the chronically mentally ill (Masters level).
• Competency in substance abuse, family support, or vocational/educational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.

Responsibilities

• Provide Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between Youth ACT and other systems.
• Provide culturally sensitive and developmentally appropriate treatment to Youth and their families across services including counseling, case management and psychoeducation.
• Maintain case records and complete documentation on time in compliance with regulations.
• Deliver intensive, in-person, community-based services calibrated to each individual's needs and adjust in real time as needed.
• Be available 24/7 with the Youth ACT Team, including after-hours coverage as needed.
• Assist recipients in developing appropriate expectations from treatment.
• Discharge planning and termination to ensure resources after treatment ends.
• Exchange information with other agencies to benefit the recipient and ensure regulatory compliance.
• Participate in supervision, staff development and NYS Youth ACT trainings.
