Job summary

Center for Life Management seeks a Young Adult Community Case Manager to support clients 15-26 with mental health needs. You’ll develop care plans, coordinate resources, monitor services and provide home or community-based counseling and support.

Role emphasizes collaboration with the treatment team, documenting results, and supporting family involvement while upholding agency standards and confidentiality.

Qualifications

• Bachelor’s degree preferred; associate’s degree with 2 years direct service may qualify.
• Ability to exercise sound clinical judgment in routine and urgent situations.
• Proficient with Microsoft Office and documenting in EHR systems.
• Ability to work within a clinical team and also work independently.

Responsibilities

• Provide care planning, resource identification, benefit coordination and monitoring.
• Deliver counseling and community-based support to assist clients’ wellness goals.
• Coordinate with treatment team, natural supports and external providers.
• Maintain timely documentation of services and outcomes.
• Engage families with training and supportive services as appropriate.
• Represent CLM professionally and adhere to agency policies.
