Professional Services Group, Inc. is seeking a Service Facilitator in the Families First Program in Kenosha, WI.

You will provide intensive case management to children and families, using wraparound services to build comprehensive treatment plans and coordinate with a wide network of professionals. The role requires a Bachelor's degree in a related field, a valid driver's license, and reliable transportation; one year of experience with severely emotionally disabled populations is preferred.

Formación

• Bachelor's degree required or equivalent in social services fields.

Responsabilidades

• Provide intensive case management services to children and families with SED.
• Use wraparound model to identify services and develop multi-disciplinary treatment plans.
• Collaborate with case managers, family advocates, mental health providers, and school staff.
