Job summary

Professional Services Group in Jefferson County, WI is seeking a Service Facilitator to deliver intensive case management to individuals with SED under the CCS wraparound model. You will collaborate with a multidisciplinary team to identify services and develop plans.

Responsibilities include creating treatment plans, forming a family support team, leading meetings, crisis planning, advocacy, attending IEPs and court events, and transporting clients as needed. This is a full-time, on-site role.

Qualifications

• Bachelor's degree required in social work, psychology, criminal justice, or related field.
• Minimum of one year experience working with the severely emotionally disabled population.
• Reliable daily transportation, valid driver's license, and automobile insurance.

Responsibilities

• Create individualized treatment plans for each assigned client.
• Develop a family support team comprised of formal and informal supports.
• Facilitate team meetings with service providers, family members, and family supports to monitor progress.
• Develop a crisis/safety plan for the client and family.
• Advocate for clients in emergency and crisis situations.
• Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other advocacy events.
• Safely transport clients as needed.
