Job summary

Northshore Home Health Care in Louisiana is seeking a Wraparound Supervisor to lead a multidisciplinary team serving at-risk youth and families. You will coordinate complex cases, supervise wraparound facilitators, and ensure trauma-informed, strengths-based care with a client-centered approach.

You will partner with schools, courts and community agencies, monitor caseloads, document outcomes, and coach staff through training and crisis guidance to maintain high service standards.
