Job summary

Eckerd Connects in Louisiana seeks a Wraparound Supervisor to coordinate care for WAA enrollees, lead planning and family involvement, and identify goals and strengths in risk assessments and plans of care.

The role supervises up to 8 facilitators, collaborates with state management and families, and provides coaching, HR tasks, and crisis support, with travel and flexible scheduling.

Qualifications

• Master's degree from an accredited university or college in human services, social, or behavioral science required.
• 3+ years of experience in the human services field required, preferably with different systems.
• 2+ years demonstrated success in management/supervision required; experience in managing community-based programs.

Responsibilities

• Ensures individualized planning, effective service coordination and management of care for all enrollees including family participation and goals development.
• Supervises and guides Wraparound Facilitators in working with child/youth and families to identify strengths, needs and goals.
• Supervises up to 8 Wraparound Facilitators and coordinates between Statewide Management Organization, facilitators and families.
• Observes and documents CFT and engagement meetings; completes observation forms.
• Reviews all documentation and provides feedback to the team.
• Performs HR tasks including time requests, referrals, documentation reviews and corrective actions.
• Assists with flex fund requests and crisis support for families.
• Conducts regular group coaching with Training Coordinator and Lead Coach for trainings.
