Qualifications

• High School Diploma or GED preferred.
• Bi-lingual/Bi-literate in Spanish highly preferred.
• Valid California Driver's License and proof of Auto Insurance required.
• Must pass all required background checks and screenings.

Responsibilities

• Adheres to the Wraparound model and CFT process.
• Attends and participates in regular Child and Family Team Meetings in home or community settings.
• Collaborates with family, clinicians, school staff and service providers to offer advocacy and support services.
• Facilitates linkage to community services and assists with intake and screening appointments.
• Leads or co-leads parent groups for current and discharged families.
• Maintains professional rapport with clients, families and regulatory bodies.
• Performs other duties as assigned by the Wraparound Coordinator/Program Manager.
